Hunt Of A Lifetime - How It Works To refer a youth to Hunt Of A Lifetime please select one of the printable application formats below. This form must be printed, filled out in it's entirety and signed by the participant or Parent/Guardian and the waiver must be signed by a Notary Public and notarized.Dear Parents and Friends, We at Hunt Of A Lifetime are happy to help your youngster with the Hunting &/or Fishing Excursion of their dreams. In order to ensure that everyone involved has a pleasant experience, please review the following and ask for clarification if there is anything you are not sure of. HOAL will provide for the youngster: THE TAXIDERMY, for a maximum of 2 animals-shoulder mount/fish, if they achieve a harvest. We also supply them with Clothing, please include their sizes. These will either arrive as package from Cabela's or a Gift Card from Gander Mountain (if gift card when you go shopping, this is for a rifle, if hunting & ALL your other needs); For the youngster and ONE companion we cover: AIRFARE, LODGING, MEALS, SHIPPING OF GAME (YOUTH'S) & RENTAL CAR (if needed) as well as any other expenses that would be incurred in helping their dream become become a reality. Additional family members &/or friends, with approval from outfitter/charter boat captain, may accompany them BUT IT IS AT THEIR OWN EXPENSE. HOAL makes flight and accommodation arrangements for all participants, with extra participants reimbursing HOAL prior to departure for this additional expense. Please provide names of participants AS THEY APPEAR ON THEIR IDENTIFICATION (NO NICKNAMES). Airlines are very strict regarding identification. If you are traveling out of the country, even to Canada, proof of citizenship is required. This must be a state-issued birth certificate with a raised seal OR passport. If you are UNSURE about your identification, please check with our travel agent: Terry Sage 814-454-5447 X 108. We would hate to see the trip ruined over lack of improper documentation. We will provide a check prior to departure for incidentals during the trip for the youth and ONE companion. These funds are for their MEALS, RENTAL CAR &/OR HOTEL EXPENSES (IF NECESSARY), FILM DEVELOPMENT & CD, VIDEO DOUBLING AND ANY OTHER REASONABLE COSTS. (AGAIN, ADDITIONAL COMPANIONS ARE AT THEIR OWN EXPENSE); WE DO NOT pay for alcohol or tobacco. RECEIPTS ARE REQUIRED! Upon your return, please remit the RECEIPTS along with any unused funds. *Please note that if a rental car is necessary for your hunt, you will need a current (age 25^) drivers' license as well as a credit card in your own name. Your credit card will not be charged (unless you choose to be reimbursed after the hunt), but a credit card is a requirement for any car rental. Included with your other items you will find a pre-paid Priority Mail Envelope FOR USE UPON COMPLETION OF THE TRIP. Please do NOT use the envelope simply to return applications or general correspondence. We will need the following: 1.) RECEIPTS and balance of unused expense funds (if we owe you, please note this, we will reimburse you) 2.) DUPLICATES OF FILM &/or video developing, including a CD of the pictures. 3.) THANK YOU NOTE from the youngster to our "Hunt of a Lifetime sponsors". 4.) LIST of any & all that may have helped you during the Hunt &/or Fishing DREAM.
